Uniform Complaint Procedure Form

The Uniform Complaint Procedure (UCP) Form is used to report concerns or complaints related to programs, services, or activities within Selma Unified School District. All questions are required to be answered. Once your form is received, the District will review your submission and contact you regarding next steps. 

You may complete the form in one of the following ways:

1. If you have a Gmail account: Complete the Google Form version and click Submit when finished.
2. If you do not have a Gmail account: Print out the form and mail, fax, or email the completed UCP Form and supporting documents to:

El Formulario de Procedimiento Uniforme de Quejas (UCP, por sus siglas en inglés) se utiliza para reportar inquietudes o quejas relacionadas con programas, servicios o actividades del Distrito Escolar Unificado de Selma. Es obligatorio responder todas las preguntas. Una vez recibido su formulario, el Distrito lo revisará y se comunicará con usted para informarle sobre los pasos a seguir.

Puede completar el formulario de una de las siguientes maneras:

1. Si tiene una cuenta de Gmail: Complete el formulario de Google y haga clic en "Enviar" al finalizar.

2. Si no tiene una cuenta de Gmail: Imprima el formulario y envíelo por correo postal, fax o correo electrónico, junto con los documentos adjuntos, a:

ਜਾਂ ਗਤੀਵਿਧੀਆਂ ਨਾਲ ਸਬੰਧਤ ਚਿੰਤਾਵਾਂ ਜਾਂ ਸ਼ਿਕਾਇਤਾਂ ਦੀ ਰਿਪੋਰਟ ਕਰਨ ਲਈ ਕੀਤੀ ਜਾਂਦੀ ਹੈ। ਸਾਰੇ ਸਵਾਲਾਂ ਦੇ ਜਵਾਬ ਦੇਣ ਦੀ 
ਲੋੜ ਹੁੰਦੀ ਹੈ। ਇੱਕ ਵਾਰ ਜਦੋਂ ਤੁਹਾਡਾ ਫਾਰਮ ਪ੍ਰਾਪਤ ਹੋ ਜਾਂਦਾ ਹੈ, ਤਾਂ ਜ਼ਿਲ੍ਹਾ ਤੁਹਾਡੀ ਸਪੁਰਦਗੀ ਦੀ ਸਮੀਖਿਆ ਕਰੇਗਾ ਅਤੇ ਅਗਲੇ 
ਕਦਮਾਂ ਬਾਰੇ ਤੁਹਾਡੇ ਨਾਲ ਸੰਪਰਕ ਕਰੇਗਾ।

ਤੁਸੀਂ ਹੇਠ ਲਿਖਿਆਂ ਵਿੱਚੋਂ ਕਿਸੇ ਇੱਕ ਤਰੀਕੇ ਨਾਲ ਫਾਰਮ ਭਰ ਸਕਦੇ ਹੋ:

1. ਜੇਕਰ ਤੁਹਾਡੇ ਕੋਲ ਇੱਕ Gmail ਖਾਤਾ ਹੈ: Google ਫਾਰਮ ਸੰਸਕਰਣ ਨੂੰ ਪੂਰਾ ਕਰੋ ਅਤੇ ਪੂਰਾ ਹੋਣ 'ਤੇ Submit 'ਤੇ ਕਲਿੱਕ ਕਰੋ।
